SAFETY WARNING

DO NOT COMMENCE OR CONTINUE EXCAVATION OPERATIONS UNLESS:

A trained Competent Person is on site and supervising all excavation work.

All personnel are trained in the use of the Poly Shore system, in proper excavation work procedures and all safety precautions.

The Competent Person has determined that working conditions are safe and that the Poly Shore system is being used within specifications, including soil type.

The Competent Person has authority to stop work when it is unsafe for workers to enter an excavation.

The excavation site and the Poly Shore system are inspected at least daily and whenever there is a change of soil, water or other job site conditions.

The installation and removal of the Poly Shore system is in compliance with all instructions, warnings and safety practices.

All lifting and pulling equipment is determined to be suitable and of sufficient capacity by the Competent Person and has been inspected for damage or defects prior to use.

THE FOLLOWING SAFETY PRACTICES MUST BE FOLLOWED AT ALL TIMES:

Personnel must adhere to all safety and training guidelines, follow all instructions and warnings and must follow the direction of the Competent Person on site.

Personnel most not enter an excavation site that is not properly shored, shielded or sloped.

Personnel in the excavation must always work within the Poly Shore system.

Personnel must not stand on the edge of an unshored excavation.

Personnel must enter and exit excavations only within shored areas.

THE TABULATED DATA IS ONLY A GENERAL SET OF GUIDELINES AND TABLES TO ASSIST THE COMPETENT PERSON IN SELECTING AND USING THE PROPER POLY SHORE SYSTEM. THE CUSTOMER AND THE COMPETENT PERSON ARE SOLELY RESPONSIBLE FOR JOBSITE SAFETY AND THE PROPER SELECTION,
INSTALLATION, USE AND REMOVAL OF THE POLY SHORE SYSTEM.

THE TABULATED DATA IS NOT A SAFETY PLAN, BUT IS TO BE USED BY THE COMPETENT PERSON IN CONJUNCTION WITH TRAINING, EXPERIENCE AND COMPLETE KNOWLEDGE OF JOB CONDITIONS AND SOIL TYPE.
